  • You must be seen in the office or on a tele-visit for the reason of disability. At that visit, the forms will be reviewed and discussed for further action.

  • We have a provider that will evaluate your needs and determine what the best plan of action is for you. Counseling sessions can be onsite.

  • Labs are available all other testing will need to be reviewed in the office and you can request a copy

  • Yes, we can set you up with a team that focuses on healthy lifestyles.

  • No that is not a service we offer directly. We do have programs we are aware of if you qualify.

  • We do not offer physical therapy in the office. We have a team of centers that we work closely with to provide therapy needs.

  • You must be seen by the provider for the services being requested. Once you and your provider review this need the process of an outgoing referral will start. This may include obtaining authorization and documents being faxed to the requested center.

  • We try to accommodate all appointment times. If we do not have the time for a same-day appointment available we offer walk-ins, just expect a small wait time.

  • We have an ARNP who specializes in women’s health care.

  • We prefer at least a 48 hour notice. If you can cancel instead of a no-show that will always be better.

  • Yes, we offer tele-visits. You must have a smartphone to do this. We also require your basic vitals for those visits too.

  • We can do refills however on controlled substances you must make an appointment to be seen for those. This cannot be a walk-in or same day appointment.

  • Yes, we can do sport physicals in our express care center. Please make sure you have the required forms. Please call to make that appointment. 352-728-4242

  • Yes please see us as soon as possible for wound care. We will clean and dress it as needed.

  • You must be seen in the office and provide any and all forms to be considered.

  • No, we do not offer that service, however, we can help you get those services started.

  • Yes, we require all documents from the provider performing the procedure. @ appointments are required for clearance in most cases.

  • We do not perform sleep studies. We will send a referral to an at-home or in-center service for this.

  • You can be seen for that form to be completed. The standard $10 charge is required at the visit. This is not billed to the insurance.

  • We do not give documentation for medical marijuana
